Bad Pay Per View feature of Direct TV

We disconnected our Direct TV services on May 31 2010. We called to be sure there were no additional charges on the account.
The Direct TV representative confirmed that the account was current and that we would receive no additional billing.
We also received a credit after returning the Direct TV equipment. Today 7/9/10 we received a bill for $21.95 for a Pay Per View Item.
We called Direct TV immediately and found the charge is from 2007. They are claiming we ordered a Game Plan on 9/29/07.
We did not connect the phone line to any of our satellite boxes and never ordered a Pay Per View item.
We have heard of the mistakes and issues with the PPV service and did not want to participate. I am unclear how they can bill us for an item that is almost 3 years old.
The Supervisor Karen is claiming it could be a malfunction in the line (that we never connected).
If a PPV can be ordered from the remote alone why is the phone line provided and what would prevent young children from accidentally ordering services?
Her explanation is that there could be many items ordered stored in the box. In addition there is not password protection.
This is truly deceptive!
