Connecting a used receiver to TV - Do I need Direct TV to come out?

I have a used Directv/Hughes receiver (SD-HBH). If I wanted to hook it to a TV without Direct TV, do I have to call Direct tv to come out to connect the proper wiring to the receiver, and get a programming card?

Will I have to pay any fees? This receiver doesn't have a remote either. Can I purchase one from Directv, or use a universal remoter from the store?
